This app is developed by the University of Alabama at Birmingham and funded by the March of Dimes. This is a questionnaire app for UAB consenting maternity patients. The app provides suggestive feedback through the IMPLICIT model questionnaire process to assist and evaluate mother's who have recently delivered their babies in improving healthy lifestyles. All information provided in this questionnaire is strictly confidential and will only be used by UAB health care providers and the corresponding data analysis teams.
